Bring a photo ID, glasses or contacts if you use them, hearing aids if you use them, a medication list, and any employer forms. If you have sleep apnea, diabetes, high blood pressure, cardiac history, or another monitored condition, bring the most relevant recent records too.
No. The standard DOT physical includes urinalysis as part of the medical exam, but employer drug testing is a separate service unless it is scheduled with the visit.
