What to Bring to a TLC Medical Exam

A TLC medical-certification visit supports the medical portion of a New York City Taxi and Limousine Commission driver application or a specific renewal requirement. It does not issue a TLC license or finish a rideshare platform’s onboarding.

New applicants and renewal drivers use different TLC medical forms. If you are renewing, bring your TLC license number too so the form stays aligned to the right renewal workflow.

Bring the requested licensing documents

Bring ID, driver-license information, the application or renewal notice, the exact form, medicines, vision aids and relevant medical records. Tell the clinic whether you are a new applicant or responding to a renewal request. Check the examination date against the correct requirement. A generic note may not be accepted. Ask whether the actual examiner can complete the accepted form before traveling.

Eligibility: TLC driver medical examination

New York City Local Law 165 of 2023 permits commission-designated physicians, physician assistants and nurse practitioners to examine taxi driver applicants. The currently linked new-driver form still uses physician-only wording. Before booking, confirm that the actual examiner and completed form will meet the current TLC acceptance process. The law broadens permitted professional types; it does not mean that every clinician or clinic is designated for the required examination.

What to expect: TLC driver medical examination

For a new application, download the official new-driver form and coordinate the exam with your application date. Bring it to the clinician, then check that the completed document contains the required signature and validation. Submit it through the TLC application or TLCUP as instructed. Check the remaining application requirements and status separately. If TLC sends a notice about missing information, respond to that specific request and deadline rather than submitting an unrelated clinic note.

Preparing for your appointment

Bring photo ID, your driver-license and application information, the requested form, a medication list, glasses or hearing aids and relevant medical records. For a new-driver application, the official form says the examination cannot be more than 90 days before the application. It also requires submission within 90 days after the application. A renewal has its own current account and notice instructions; do not apply the new-applicant exam window to a renewal without checking them.

Risks and when to seek help

Discuss conditions or medicines that may affect alertness or safe vehicle operation. Do not stop treatment, omit a prescription or drive while impaired to try to finish licensing faster. The clinician may need another record or assessment before signing. A completed medical form does not permit driving with an expired or unapproved TLC license. Follow any clinical restrictions and use TLC’s official license-status information before taking passengers.

Follow-up and treatment review

TLC currently permits renewal up to 90 days before expiration. An expired license stays expired until TLC processes and approves a complete late renewal, even where TLC allows requirements to be completed within 180 days after expiration. These are licensing windows, not proof of a universal medical-exam deadline. Check your TLC account or renewal notice, and ask TLC whether a medical form is required and which date rule applies to your renewal.

Appointments, costs and coverage

The listed self-pay TLC medical visit costs $125. Nao has 11 local clinics. Confirm appointment availability, examiner eligibility and any extra charge before travel. TLC drug testing must follow TLC’s approved-site process, currently described through Labcorp. A Nao workplace collection or a home test does not automatically satisfy that process. Ask TLC about the exact test requirement and fee; the clinic medical-visit price does not include every city licensing or drug-testing cost.

Do I need to print the TLC form before the visit?

Use the TLC Medical Certification Form for the correct new-applicant or renewal pathway, obtained from TLC’s current forms directory or your account instructions. Bring the current required form and application information to the clinic. A general physical note is not a substitute. The forms directory has inconsistent new/renewal links, so confirm the correct document with TLC if the listed PDF is unavailable or mismatched.

What should renewal drivers bring that new applicants may not need? Should I bring my TLC application receipt?

Bring your DMV license, TLC license or application details, the correct current medical form, medicine list, glasses/hearing aids and relevant medical records. Renewal drivers should also bring their renewal notice and account instructions. A new-applicant payment receipt may be needed for the separate TLC drug-test pathway. Ask intake which documents your medical appointment requires instead of treating the drug-test checklist as identical.

Do I bring drug-test paperwork to the medical exam?

The TLC medical form and TLC drug testing are separate requirements. Follow TLC’s current drug-testing instructions and approved collection pathway, rather than assuming a general clinic urine test satisfies it. Confirm applicant/renewal status, due dates, required identification and the exact authorized collection location with TLC.

Can I do the TLC drug test at the medical clinic?

Follow TLC’s approved drug-testing site and booking instructions. Its current program uses the Labcorp pathway. A medical exam, general workplace collection or home test does not automatically satisfy the TLC drug-test requirement.
