Care for childhood illness and minor injuries

Pediatric urgent care can assess a child with a stable illness or minor injury. Tell the clinic the child’s age, symptoms and how long they have been present, and confirm its pediatric age limits before traveling. School and sports physicals are separate appointments with their own forms and examination requirements. A baby younger than three months with a temperature of 100.4°F (38°C) or higher needs prompt medical assessment. Severe breathing trouble, unresponsiveness, a seizure or another life-threatening symptom needs emergency care.

School, camp and sports physicals

A school, camp or sports physical reviews a child's health for a particular activity or requirement. These forms are not interchangeable. A sports assessment considers participation risks; a school examination documents required health information; a camp may require its own form and medication plan. Bring the exact document so the clinician can determine what is needed.

Confirm the child's age, form type and submission deadline. New York school requirements depend on grade, status and current state rules. Sports eligibility also depends on the school or athletic program's rules and interval health changes. A routine wellness visit does not automatically meet every clearance requirement.

What to expect: School, camp and sports physicals

The clinician reviews the form, medical and family history, medications, vaccines and relevant examination findings. A concerning answer may require records, testing or specialist assessment before a decision. The clinician may complete a form when sufficient information is available, defer a decision or describe restrictions. Completion cannot be promised before examination.

Preparing for your appointment

Bring the current required form, vaccine record, prior relevant assessments, glasses or hearing aids and a medication list. Include an asthma action plan or emergency medication information if applicable. Ask whether a parent or guardian must attend and whether permission is needed for a minor's visit. Do not fast unless a particular ordered test requires it.

Risks and when to seek help

Chest pain or fainting with exercise, unexplained severe shortness of breath, a recent concussion or a significant unresolved injury needs assessment before participation. Do not hide symptoms to meet a deadline. New severe symptoms require urgent or emergency care even if a form was recently signed.

Follow-up and treatment review

Give the completed document to the requesting program and keep a copy. Ask how restrictions, pending referrals or medication forms will be handled. A change in health after the examination may require another review. A positive screening answer is a reason to investigate, not an automatic diagnosis or permanent ban from sport.
