Plan for PrEP costs and coverage

Your out-of-pocket cost depends on the medicine, benefits, clinical services and any assistance you qualify for. Ask for a clear estimate before starting.

Coverage review and program applications do not guarantee approval or a zero-dollar visit.

Possible HIV exposure within 72 hours?

If you may have been exposed to HIV within the past 72 hours, seek urgent medical evaluation now. PEP works best when started as soon as possible. Do not wait for a routine PrEP appointment; go to an emergency department if prompt evaluation is unavailable.

Urgent PEP guidance

Ask about each part of the bill

Visits and tests

Ask about the initial evaluation, HIV and other lab work, and follow-up visits. Check the network status of both the clinic and laboratory.

Prescription medicine

Check which PrEP option your plan covers, prior authorization, preferred alternatives, specialty pharmacy requirements and refill timing.

Injection administration

For injectable PrEP, ask how the medicine and administration are billed, which clinic is covered and whether separate cost sharing applies.

If you have insurance

Have your medical and pharmacy cards ready. Ask your insurer about your exact plan, deductible, copay or coinsurance, and whether authorization is needed for the selected regimen. A general statement that an insurer is accepted does not establish coverage for a particular medicine or visit.

Ask what happens if authorization is delayed or denied, and how to maintain prevention while the clinician and team review alternatives.

If you are uninsured or need help

New York State and manufacturer assistance programs may help eligible patients with PrEP-related services or medicine. Eligibility, documentation, covered services and enrollment rules differ. Visit, lab and medication assistance may come from different programs.

Start with NYC Health’s PrEP resources and ask the clinic which current assistance pathways may apply. Do not assume Nao participates in every listed program or that an application is approved.

Bring these questions to the visit

What will the evaluation and lab work cost? Which pharmacy will supply the medicine? Is the injection clinic in network? Who will explain a denial? What is my follow-up schedule, and how do we avoid a gap if coverage changes?

Questions to discuss before your visit

Is PrEP always free?

No single cost applies to everyone. Benefits, the selected regimen, services billed and assistance eligibility must be checked.

Can I use manufacturer copay assistance with any insurance?

Program rules differ, and government insurance can affect eligibility. The program must confirm whether you qualify.

Should cost questions delay PEP after an exposure?

Seek urgent medical evaluation after a possible HIV exposure. Do not wait for routine PrEP coverage review when PEP may be needed within 72 hours.
