This checklist helps you prepare. The ordering organization must still provide the required authorization, clinical order or custody-and-control paperwork.
What does “a work physical” actually include?
The employer’s requirement defines the visit. A general physical does not automatically include a drug test, TB screening, titers, vaccines or every employer form. List each required item so the clinic can confirm the service and the price inclusions.
Specialized credentials can also require a particular examiner or form. A DOT/CDL physical, Coast Guard medical examination and OEUK assessment are different requirements. Confirm the exact certificate and qualified examiner before booking.
Give the employee one clear set of instructions
Combine the service name, clinic, appointment details, order reference and payment instructions in one message. Include the person to contact if the order is missing or the appointment needs to change.
Use the clinic’s preparation instructions for the specific service. Employees should not fast or change medication unless the treating clinician or testing team instructs them to do so.
Use blank forms when discussing a new employer program. Send employee medical information only through the appropriate clinical or reporting channel.
If the visit cannot be completed
Ask which item is missing: an order, a form, a test, a signature or an authorization. Identify who can supply it and confirm the correction before arranging another visit.
A missing employer document may not require another examination. Likewise, a completed examination may still have laboratory results pending. Use the results and status guide to help identify the next step.

Is a screenshot of a job requirement enough?
It can help explain the requirement, but it may not provide the order, authorization or reporting instructions the clinic needs. Ask the ordering organization and clinic for the required paperwork.
Does this checklist authorize a DOT drug test?
No. Use the required program order and paperwork. This preparation checklist is not a Federal Custody and Control Form.
Who should pay for the visit?
Clarify the employer’s billing arrangement before the employee books. If the employee will pay, confirm the exact service, price inclusions and any reimbursement process with the employer.
Do employees need to book a meeting with the employer team?
No. Employees use the appropriate patient booking or clinical contact. The employer team helps organizations set up services, billing and reporting.
