TREMFYA injections and infusions: know your prescribed regimen

The TREMFYA schedule depends on the condition. Before your first dose, confirm the diagnosis, strength, device, route and next appointment or refill date. This overview supports a discussion with the care team; use the instructions supplied with your actual product.

An appointment does not confirm access to a particular medicine, procedure or specialist. Ask the team about the care and coverage you need.

Medical information checked against current US sources: October 19, 2026.

Plaque psoriasis and psoriatic arthritis

The labeled dose for eligible skin and joint disease is 100 mg subcutaneously at week 0, week 4 and every eight weeks afterward. It is not an IV regimen. The age and weight requirements are at least six years and at least 40 kg.

Adults can self-administer after appropriate training under professional guidance. For pediatric patients, the label recommends administration by a healthcare professional or trained caregiver rather than the child. Device instructions and permitted injection sites can differ by age and presentation.

Adult Crohn’s disease and ulcerative colitis

Induction is either 200 mg intravenously over at least one hour at weeks 0, 4 and 8, or 400 mg subcutaneously at the same weeks. Each 400 mg dose uses two consecutive 200 mg injections. An IV vial and a subcutaneous pen are different products and must not be substituted.

Maintenance is subcutaneous: 100 mg at week 16 and every eight weeks afterward, or 200 mg at week 12 and every four weeks afterward. The clinician selects the lowest effective recommended maintenance regimen. Ask for a calendar that includes the transition, not just the first three doses.

Prepare for safe injection training

The label describes thighs and the abdomen, avoiding the area within two inches of the navel. Upper-arm use depends on the person giving the dose, age and exact device. Do not inject into tender, bruised, red, hard, thick, scaly or psoriasis-affected skin. Ask the trainer to demonstrate the instructions for your prescribed pen, One-Press injector or syringe.

Take the product out of the refrigerator and allow the labeled 30-minute warming period, up to 25°C/77°F, without removing the needle cap. Do not heat or shake it. Check the liquid and packaging as described in the Instructions for Use and use each device only once.

Storage, travel and a dose that does not go as planned

Store TREMFYA refrigerated at 2–8°C/36–46°F in its original carton, protected from light. Do not freeze or shake it. If it has been left out longer than the instructed preparation period, overheated, frozen or delivered warm, ask the dispensing pharmacy before using it. Do not assume a room-temperature allowance from another biologic applies.

If an injection is incomplete, leaks or the device fails, contact the prescribing team or pharmacy. Do not repeat a full dose because you are unsure how much entered. For IV treatment, ask the site about its preparation and total visit time; the minimum one-hour infusion is not a promise of a one-hour appointment.

Questions patients ask

What if I miss a TREMFYA dose?

The label says to administer a missed dose as soon as possible, then resume the regular schedule. Contact the prescriber or pharmacy if you are unsure about the dose, a long interruption, an infection or where the next date belongs. Do not change the maintenance interval or give extra medicine yourself.

Where should TREMFYA be injected?

Follow the Instructions for Use for your exact product and age group. Thigh and abdominal sites are described in the label, with restrictions around the navel and affected skin. Upper-arm administration has caregiver, healthcare-professional and device-specific rules. Obtain training rather than copying a video for another device.

How long can it be out of the refrigerator?

The label describes a 30-minute preparation period up to 25°C/77°F before injection. That should not be treated as a general travel or extended-storage permission. For an unexpected temperature exposure, tell the pharmacist the product, temperature and duration before deciding whether to use it.

Can I give an IV dose at home myself?

No. The IV product requires professional dilution, preparation and administration. A trained adult may use a prescribed subcutaneous product after instruction, but that is a different route. Never put medicine from a pen or syringe into an IV line.

Why are my starting and maintenance doses different?

For adult bowel disease, induction is designed to start treatment and maintenance is the ongoing regimen. The labeled doses, routes and first maintenance dates differ. Confirm the calendar with the prescriber and pharmacy so an approved starting course is followed by the correct ongoing prescription.
