TREMFYA for Crohn’s disease: induction, maintenance and follow-up

TREMFYA is FDA-approved for adults with moderately to severely active Crohn’s disease. Choosing it involves more than selecting an infusion center: your gastroenterology team should confirm the treatment goal, route, tests, pharmacy and ongoing monitoring.

An appointment does not confirm access to a particular medicine, procedure or specialist. Ask the team about the care and coverage you need.

Medical information checked against current US sources: October 19, 2026.

Confirm the disease and the goal

Bring the diagnosis, scope or imaging reports, relevant laboratory results, past biologics and the reason for a change. Crohn’s disease can affect different areas of the digestive tract. Treatment should address your pattern of inflammation, complications, prior response and current health.

Medicines can help bring on and maintain remission, but they do not cure Crohn’s disease. Symptom relief alone does not always establish that inflammation has healed. Ask what symptoms, stool or blood markers, endoscopy or imaging will be used to review response.

Two labeled ways to begin

Adult induction can use 200 mg by intravenous infusion over at least one hour at weeks 0, 4 and 8, or 400 mg subcutaneously at the same weeks. Each 400 mg subcutaneous dose is two consecutive 200 mg injections. The prescriber selects the route; the products are not interchangeable administration instructions.

Maintenance is subcutaneous: either 100 mg starting at week 16 and every eight weeks afterward, or 200 mg starting at week 12 and every four weeks afterward. The label directs use of the lowest effective recommended maintenance dose. Confirm the exact first maintenance date before finishing induction.

Tests and safety before starting

The label requires TB evaluation before TREMFYA. For Crohn’s disease, obtain liver enzymes and bilirubin before treatment, monitor for at least the first 16 weeks, and continue periodic checks afterward. Review active infections, prior TB treatment, vaccines, liver disease and other immune-active medicines.

Report fever, worsening infection symptoms, yellowing of the eyes or skin, dark urine or other new concerning symptoms promptly. Severe abdominal pain, persistent vomiting, heavy bleeding, fainting or inability to stay hydrated needs urgent medical assessment, not a routine booking or a wait for the biologic to work.

Avoid a gap between induction and maintenance

Ask whether each step falls under the medical or pharmacy benefit, who requests authorization, which pharmacy supplies the product and where it will be given. An approved infusion series does not automatically establish coverage for the later injection prescription.

Confirm any administration appointment with the actual treatment site. A general Nao visit is not a confirmed gastroenterology appointment or infusion reservation. Do not ship a specialty medicine to a clinic until that clinic has accepted the prescription and delivery arrangement.

Questions patients ask

Can I start TREMFYA for Crohn’s without an infusion?

The adult Crohn’s label includes a fully subcutaneous induction option: 400 mg, given as two consecutive 200 mg injections, at weeks 0, 4 and 8. The alternative is the IV induction regimen. Your gastroenterologist chooses the route and confirms training and access; do not substitute one product for another yourself.

Will I continue getting IV infusions long term?

The labeled Crohn’s maintenance regimens are subcutaneous. If you begin with IV induction, plan the transition to the prescribed injection dose and date. The label does not provide an IV maintenance regimen.

How quickly should my bowel symptoms improve?

Your team should set a review based on symptoms and objective disease activity. Clinical study endpoints are group assessments, not a personal promise. Worsening pain, fever, bleeding or dehydration requires earlier evaluation rather than waiting for a scheduled response check.

Can children take TREMFYA for Crohn’s disease?

The current US Crohn’s indication is for adults. Pediatric approval for skin or joint disease does not establish safety, effectiveness or a dose for pediatric Crohn’s. A pediatric gastroenterologist should direct treatment decisions for a child.

Should I stop steroids when I begin?

Do not stop or taper corticosteroids on your own. The gastroenterology team should provide a specific plan that accounts for disease activity, the steroid dose and how long you have taken it. Starting a biologic does not create an automatic safe taper schedule.
