Your first PCP visit
Bring your medicines and relevant records. Discuss your health history, current concerns, and a follow-up plan.
A Healthfirst card is the starting point for verification. Have the full plan name ready, including whether you have Medicaid Managed Care, an Essential Plan, a Leaf plan, or Medicare Advantage. The team needs to check the product and provider rather than rely on the insurer’s name alone.

Bring your medicines and relevant records. Discuss your health history, current concerns, and a follow-up plan.
Review screening needs, health goals, and recommended vaccines with a primary care clinician.
Discuss ongoing conditions, recent test results, or changes in your health. Ask what the next appointment should cover.
Plan enrollment and provider assignment are separate from booking a visit. Confirm both before relying on in-network benefits. If your card identifies a Medicaid managed care plan, the Medicaid primary care guide explains what to check. For other insurers, use our primary care insurance guide.
Choose a clinic that works for your regular visits. Confirm the provider’s participation in your exact plan and whether they are accepting new primary care patients.
