Aristada Initio is not a recurring maintenance injection and is not interchangeable with Aristada. The current label describes one 675 mg Aristada Initio injection with one 30 mg oral aripiprazole dose in connection with the first Aristada injection. Nao can coordinate an accepted outside psychiatrist's initiation plan or provide psychiatric evaluation and management when needed. The complete prescriber-led sequence is confirmed before administration.
Evaluation comes before treatment
Two ways to coordinate care: If an outside psychiatrist prescribes and manages the treatment, Nao can review an accepted order and records for in-clinic administration. If the patient needs Nao to manage treatment, Nao psychiatry can evaluate, prescribe when appropriate, and provide follow-up. Product, order, delivery, staff, and site are confirmed separately.
The initiation sequence must be treated as one plan
Aristada Initio has a narrow product-specific role. A fragmented order or an assumption that Initio replaces Aristada can create a medication error.
Approved role
Aristada Initio, with oral aripiprazole, initiates Aristada when Aristada is used to treat schizophrenia in adults.
Single labeled dose
The label describes one 675 mg intramuscular Aristada Initio dose and one 30 mg oral aripiprazole dose.
Aristada timing
The first Aristada injection may be given on the same day as Aristada Initio or up to 10 days later under the documented plan.
Not interchangeable
Aristada Initio and Aristada have different pharmacokinetic profiles and cannot be substituted for each other.

2. Verify oral aripiprazole history
Document prior tolerability or the prescriber's plan to establish it, plus CYP2D6 status and interacting medicines that can make Initio inappropriate.
3. Match every component
Confirm the oral aripiprazole dose, Aristada Initio order, first Aristada dose, allowed timing, separate injection sites, and responsible prescriber.
4. Coordinate delivery and administration
Confirm benefits, patient-specific delivery, medication arrival, trained staff, administration site, and follow-up before scheduling the injection visit.
What to prepare for the first evaluation
Complete records reduce avoidable delays and help the clinician separate a new decision from continuation of an established plan.
- The signed orders for Aristada Initio, oral aripiprazole, and Aristada, including doses and intended dates.
- Documentation of prior oral aripiprazole exposure and tolerability.
- Current medicines, allergies, CYP2D6 information if known, and recent psychiatric or hospital records.

Safety, monitoring, and treatment boundaries
The clinical plan is individualized. These points highlight questions that need explicit review rather than replacing the current label, a full evaluation, or emergency care.
Interaction limits
The label says to avoid Aristada Initio in known CYP2D6 poor metabolizers and with strong CYP2D6 or CYP3A4 inhibitors or strong CYP3A4 inducers.
Separate injection sites
When Initio and Aristada are given on the same day, they should not be injected into the same deltoid or gluteal muscle.
Antipsychotic monitoring
Review movement symptoms, restlessness, weight, glucose, lipids, blood pressure, sedation, impulse-control changes, and pregnancy considerations.
No self-directed restart
A late Aristada dose may have more than one labeled supplementation option. The prescriber must select the correct re-initiation plan.
When to get urgent or emergency help
Get urgent help for high fever with severe muscle stiffness or confusion, fainting, a seizure, trouble breathing, a serious allergic reaction, uncontrolled movements, or immediate safety concerns.
