Tirzepatide

Tirzepatide is the ingredient in Mounjaro and Zepbound. Mounjaro is approved with diet and exercise to improve glucose control in adults and children age 10 and older with type 2 diabetes. Under its August 2026 US label, it also reduces the risk of major cardiovascular events in adults with type 2 diabetes who are at high risk. These events are cardiovascular death, nonfatal heart attack or nonfatal stroke. That cardiovascular indication is for adults. Zepbound has separate approvals for adult weight management and moderate-to-severe obstructive sleep apnea in adults with obesity. These approvals do not extend to compounded tirzepatide.

Eligibility: Tirzepatide

Zepbound weight treatment is for adults with obesity or overweight and a weight-related condition. The OSA indication needs the specified diagnosis and obesity. Mounjaro’s pediatric diabetes approval does not create a pediatric Zepbound weight indication.

What to expect: Tirzepatide

Review the diagnosis, medicine history and risks. Both approved tirzepatide brands start at 2.5 mg once weekly, but follow their separate schedules. The August 2026 Mounjaro label permits 2.5 mg increases after at least four weeks on the current dose if additional glucose control is needed. Its maximum is 15 mg weekly for adults and 10 mg for pediatric patients. Zepbound starts at 2.5 mg weekly for four weeks, then 5 mg. Its maintenance dose depends on weight or OSA treatment and tolerability. Increase only under the prescriber’s plan. Train for the exact vial or pen received and arrange monitoring and refills.

Bring your weight and treatment history, medicines and supplements, relevant glucose, kidney, and lipid results, and the exact product you have used before. Mention previous side effects, eye disease, eating difficulties, and pregnancy plans. If insurance denied treatment, bring the denial and plan details.

Risks and when to seek help

Review a personal or family history of medullary thyroid cancer, MEN 2, severe stomach-emptying problems, pancreatitis, gallbladder disease, allergies, and pregnancy plans before treatment. Nausea, vomiting, constipation, or diarrhea can occur. Persistent severe abdominal pain or inability to keep fluids down needs emergency department assessment now. Call 911 for breathing difficulty, a severe allergic reaction, loss of consciousness or unsafe travel. Tell the anesthesia team about treatment before a procedure. Do not combine GLP-1 medicines or change the dose yourself. Use a non-oral contraceptive method or add a barrier method for four weeks after starting tirzepatide and after each dose increase. Do not overlap tirzepatide products or another GLP-1 medicine without a clinician’s plan.

Follow-up and treatment review

Review weight trend, appetite, food and fluid intake, bowel symptoms, and treatment tolerability. Check glucose and adjust other diabetes medicines with the prescriber when relevant.
