Bronchiectasis care

Bronchiectasis is lasting damage and widening of the airways that makes mucus harder to clear. Repeated infections and inflammation can reinforce each other. Care aims to reduce flare-ups, clear mucus and treat underlying causes. Non-cystic fibrosis bronchiectasis has a different treatment pathway from cystic fibrosis, asthma and a cough without a confirmed diagnosis.

How bronchiectasis care works

Airway-clearance exercises and devices help move mucus. Antibiotics treat selected infections; sputum results help guide the choice. Brinsupri is an oral DPP-1 inhibitor that reduces activation of inflammatory neutrophil enzymes. It is FDA approved for non-cystic fibrosis bronchiectasis from age 12. It does not immediately dissolve mucus or replace antibiotics for an acute infection.

Eligibility: Bronchiectasis care

A lung specialist reviews the chest CT, sputum history, lung function, exacerbations and possible underlying conditions. A chronic cough alone is not enough to start Brinsupri. The clinician also distinguishes persistent infection, cystic fibrosis and other causes. Treatment choices depend on the person’s flare-up burden and existing airway-clearance plan.

What to expect: Bronchiectasis care

Bring prior imaging and culture reports to the evaluation. Review a daily clearance routine and an action plan for increased sputum, fever or worsening breathing. If an oral treatment is appropriate, coordinate the prescription, pharmacy and coverage. Continue prescribed clearance and infection treatments while benefit is assessed. Referral to pulmonology or respiratory therapy may be needed.

Preparing for your appointment

Bring CT images or access instructions, sputum cultures, antibiotic courses, hospital records and current inhalers. Describe the amount and color of sputum, any blood, and how often symptoms flare. For Brinsupri, review dental health, skin problems and vaccinations before treatment; dental or skin follow-up may need a separate appointment.

Risks and when to seek help

Call for a new flare rather than waiting for a scheduled medication review. Severe breathlessness, significant coughing of blood, chest pain or confusion need urgent or emergency assessment. Brinsupri can cause skin problems and gum or dental adverse reactions. Report a new rash, painful gums or dental changes; avoid live attenuated vaccines during treatment.

Follow-up and treatment review

Follow-up examines flare frequency, antibiotics, mucus clearance, activity and lung function where indicated. Report persistent infection symptoms even if taking Brinsupri. The clinician reviews skin and dental effects and the value of ongoing treatment. There is no promised cure or immediate mucus clearance, and an improvement does not mean airway-clearance treatment can be stopped.

Appointments, costs and coverage

Confirm whether the chosen clinic provides the evaluation, testing, specialist referral or prescription follow-up. CT, sputum testing, pulmonary rehabilitation and drug dispensing may involve other facilities. Bring supporting records for authorization, but approval is not guaranteed. Obtain separate estimates for visits, tests and medicine and confirm the local appointment type.

Questions about your care

Where is Nao Medical Astoria?

The Astoria clinic is at 37-15 23rd Ave, Astoria, NY 11105. Call the selected clinic to confirm today’s hours, the exact service and the appointment. An address does not prove onsite medication, testing or procedure availability.

Can I walk in for non-cystic fibrosis bronchiectasis care?

A consultation starts with the clinical review and required records. Treatment or a procedure depends on suitability, the actual order, safety checks, staff, supplies and any authorization; booking does not guarantee a dose or procedure that day.

Can I bring records from another clinician?

Bring CT images or access instructions, sputum cultures, antibiotic courses, hospital records and current inhalers. Describe the amount and color of sputum, any blood, and how often symptoms flare. For Brinsupri, review dental health, skin problems and vaccinations before treatment; dental or skin follow-up may need a separate appointment.

Can Brinsupri treat any chronic cough?

A lung specialist reviews the chest CT, sputum history, lung function, exacerbations and possible underlying conditions. A chronic cough alone is not enough to start Brinsupri. The clinician also distinguishes persistent infection, cystic fibrosis and other causes. Treatment choices depend on the person’s flare-up burden and existing airway-clearance plan.

Can I stop airway-clearance treatment?

Do not stop prescribed airway-clearance treatment because Brinsupri starts. Airway clearance helps remove mucus, while Brinsupri has a different role in ongoing inflammation and exacerbation reduction. Review any change with the treating lung team, especially if mucus, breathlessness or flare frequency worsens.

Will Brinsupri immediately clear my mucus or cure bronchiectasis?

No. Brinsupri is ongoing treatment intended to reduce bronchiectasis exacerbations; it does not cure the underlying disease or guarantee immediate mucus clearance. Continue the airway-clearance plan and track flares, symptoms and adverse effects. A new flare may need assessment and antibiotics rather than waiting for Brinsupri to work.

Can Brinsupri replace antibiotics during a flare-up?

No. Brinsupri is ongoing treatment to reduce bronchiectasis exacerbations; it does not replace assessment and antibiotics when a flare requires them. Contact the lung team for increased breathlessness, sputum changes, fever or other worsening symptoms. Continue the airway-clearance and flare plan. Severe breathing trouble or significant coughing of blood needs urgent assessment.

Do dental symptoms matter during treatment?

Yes. Brinsupri can cause gum and periodontal problems. Report gum swelling, bleeding, pain, loose teeth or other new dental symptoms to the treating team and dentist, and keep up oral hygiene and dental review. Those symptoms should be assessed during treatment rather than treated only as a preparation issue.

What helps with Brinsupri authorization?

Ask the plan which records it requires for Brinsupri, such as the non-cystic fibrosis bronchiectasis diagnosis, age, flare history and prior treatment. The prescribing team can submit the relevant information, but the insurer decides approval. Keep airway-clearance and other prescribed care in place while the access plan is reviewed.
