Xolair therapy is initiated in a healthcare setting equipped to manage anaphylaxis. The clinician sets observation and any later self-administration plan from the label, treatment history, and individual risk.

The care team verifies the exact condition being treated, prior therapy, current prescription, dosing inputs, and the safest administration plan.

The FDA-labeled indication

Xolair has labeled uses for certain patients with allergic asthma, chronic rhinosinusitis with nasal polyps, IgE-mediated food allergy, and chronic spontaneous urticaria.

The dose inputs

For asthma and food allergy, pretreatment total IgE and body weight help determine the labeled dose. CSU dosing does not depend on total IgE or body weight.

The administration setting

Prior reactions, treatment history, device, patient or caregiver capability, observation needs, and clinic protocol affect where and how a dose is given.

1. Confirm the diagnosis and prior treatment

Review the labeled indication, symptom control, prior medicines, relevant tests, emergency history, and why Xolair is being considered or continued.

2. Review the prescription and dose

The prescriber confirms the product, dose, interval, current weight when relevant, pretreatment IgE when required, and whether the order is new, continuing, or transferring.

3. Complete coverage and delivery steps

The care team reviews the medical or pharmacy benefit, authorization requirements, expected cost, specialty-pharmacy route, and eligible manufacturer-support resources.

4. Confirm the receiving clinic

Nao accepts the patient-specific shipment only after the location, storage, staffing, emergency readiness, and appointment plan are approved.

5. Administer and reassess

The clinical team completes pre-dose checks, administers the injection under the approved protocol, observes as directed, documents the dose, and arranges response and reorder follow-up.

The condition determines the Xolair pathway

A general Xolair request is not enough to establish eligibility. The clinician applies the current prescribing information to the diagnosis and treatment history.

For allergic asthma, Xolair is an add-on option for certain patients age 6 and older with perennial aeroallergen reactivity and symptoms inadequately controlled with inhaled corticosteroids. It is not a rescue treatment for acute bronchospasm or status asthmaticus.
For IgE-mediated food allergy, Xolair may reduce reactions from accidental exposure in certain patients age 1 and older. Patients must continue allergen avoidance, and Xolair is not emergency treatment for an allergic reaction.
For CSU, Xolair is labeled for patients age 12 and older who remain symptomatic despite H1 antihistamine treatment. It is not labeled for other forms of urticaria.
The current label contains separate requirements for initiating treatment in a healthcare setting and for selecting patients who may later self-administer.

Coverage, outside orders, and specialty-pharmacy delivery

Outside-order review

A valid order is reviewed with the diagnosis, dose, records, responsible prescriber, adverse-event plan, refill responsibility, and follow-up. Acceptance is not automatic.

Benefits and authorization

The care team can review the pharmacy or medical benefit, documentation, prior authorization, appeal path, expected patient cost, and eligible support resources.

Confirmed delivery and appointment

A specialty pharmacy may send patient-specific medication only after Nao provides the accepted clinic and delivery instructions. Administration is scheduled after receipt and safety confirmation.

Xolair safety questions to review before treatment

Tell the clinician about any prior severe reaction to Xolair or its ingredients and any history of anaphylaxis from another cause.
Call 911 for trouble breathing, throat or tongue swelling, fainting, or other signs of anaphylaxis after a dose.
Do not use Xolair as a substitute for an epinephrine plan, rescue inhaler, or emergency care.
Discuss pregnancy or breastfeeding, parasitic-infection risk, current medicines, and planned treatment changes before a dose.
