Arrange physicals for a fleet

Give the clinic the driver roster, employer authorization, billing instructions and exact examination request. Drivers still need an individual examination by a registered medical examiner and accurate medical history. Program coordination cannot guarantee certification or override a restriction. Agree on how visit status, certification and missing-record follow-up are reported. Keep DOT drug and alcohol testing orders separate from the medical examination.

DOT and CDL physical examination

A DOT physical evaluates whether a commercial driver meets the applicable FMCSA medical qualification requirements. A medical examiner on the National Registry performs the examination and makes the certification decision. Bring your license and tell the clinic whether you hold a CDL, CLP or another commercial-driver category. The medical certificate, your state license and an employer drug test are separate parts of the driver workflow.

How DOT and CDL physical examination works

The examiner reviews your health history, medications and ability to perform commercial driving duties. The examination includes measurements and assessments such as blood pressure, vision, hearing and urinalysis, with additional evaluation when the findings require it. Urinalysis used in the physical checks medical findings such as glucose or protein. It does not automatically complete a DOT drug screen, which needs its own order, collection documents, laboratory and review process.

Eligibility: DOT and CDL physical examination

The examiner applies the current standards and medical judgment to your history. A diagnosis or prescription does not always prevent certification, and a brief visit cannot guarantee it. Drivers with conditions that could affect alertness, vision, cardiovascular function or safe vehicle operation may need records from their treating clinician. The 2024 Medical Examiner’s Handbook replaces older handbook editions, including the earlier sleep-apnea bulletin. Do not rely on an old online checklist as your personal clearance rule.

What to expect: DOT and CDL physical examination

Confirm that a registered examiner will be available at the chosen clinic. Complete the health-history section accurately. At the visit, discuss current symptoms, diagnoses, medicines, prior certification restrictions and any recent changes. The examiner performs the evaluation and explains whether certification, further information or another medical step is needed. If you receive certification, check your identity and expiration details and confirm how the result reaches your state driver record.

Preparing for your appointment

Bring your license, prior medical certification, medication names and doses, glasses or contacts and hearing aids you use. Ask in advance which specialist records or sleep-apnea treatment information the examiner needs. If you use insulin, your treating clinician must complete MCSA-5870. The certified examiner must receive that form and begin the examination no later than 45 calendar days after the treating clinician signs it. Bring the requested glucose records rather than assuming a general diabetes letter replaces the form.

Risks and when to seek help

Continue prescribed treatment unless your treating clinician gives another instruction. Report fainting, severe sleepiness, chest symptoms, episodes of low blood sugar and medication effects honestly. Do not drive while a condition makes driving unsafe or when certification is required and has expired. A certificate can be limited to a shorter period than the usual maximum of 24 months. Passing an earlier examination does not establish that a new symptom is safe to ignore.

Follow-up and treatment review

For CDL and CLP holders, medical examination results are transmitted through the National Registry to the state licensing agency. Check that your licensing record has updated and keep any documents the examiner provides. An October 8, 2026 federal notice ends reliance on paper MEC proof for drivers licensed outside Alaska from October 12, 2026. The renewed 60-day paper exemption applies only to Alaska-issued CDL or CLP holders and their employers. It does not extend a certificate’s expiration or waive medical qualifications.

Appointments, costs and coverage

The listed standard self-pay DOT physical costs $135. Nao has 11 local clinics. Confirm the examiner’s availability, the price scope, employer authorization and any separate test or follow-up charge. A walk-in or same-day appointment describes access to the visit. Missing records, medical findings or a state-record delay can still affect completion. Employer billing and reimbursement depend on the actual authorization rather than an employer’s name alone.

Can employers coordinate DOT physicals through Nao Medical?

Can DOT physicals be connected to DOT drug testing support?

The DOT medical examination and DOT drug/alcohol testing are separate services. A medical urine test checks findings such as glucose or protein; it is not the required employment drug screen. DOT testing needs an authorized reason/order, the applicable collection and laboratory process, and MRO or alcohol-test review. A DOT return-to-duty process after a violation also requires the qualified SAP pathway; an ordinary physical cannot clear that requirement.

Bring the authorized requirement, relevant records and forms. Examination completion, certification and test reporting may take longer than the initial visit.

How do employers book a Corporate Health consultation?

An employer consultation defines the organization's program, sites, authorizations, billing and reports. It is different from an employee's clinical appointment. Employers should send the service list, roles, headcount, locations and timeline to the employer-service team. Employees needing an individual visit should use the relevant patient intake path and employer authorization. Urgent clinical care should not wait for a sales or program-design meeting.

Is the DOT physical urine sample a drug test?

The medical urinalysis and a DOT drug test serve different purposes. A drug test needs a separate authorized order and the applicable collection, laboratory and MRO process. Ask the employer which services are required before choosing an appointment.
